Fixed teeth in a day — All-On-4™ in London's implant centre

This scientifically established treatment concept means fixing full arch bridges with implants in a single day in patients who have either lost all their teeth or are at risk of doing so.

A special way of placing them makes this treatment concept possible. The dental implants can be securely inserted into the remaining jaw bone, often without bone augmentation. Following this a full arch bridge or denture is fixed to the implants during the surgery. You will be able to smile and eat again as normal the same day. This concept has been proven by studies that show a 99 per cent denture survival rate over an 11 year period.

Preparation

Your new teeth will be prepared according to planning models of your upper and lower jaws, photos and 3D x-ray imagery. Your first fixed temporary denture will be prepared at our dental laboratory for the day of your surgery.

On the day of your surgery

Around 8:30am: arrival and registration at the practice.

Around 9:00am: start of the sedation initiated by our anaesthetists.

Around 9:30am: surgery under sedation. The time the surgery takes may vary according to factors such as complexity and extent of the surgery.

Around 11:00am: ending the sedation and recovery in our recovery room.

Around 3:00pm: inserting the fixed denture.

Around 5:00pm: leaving the dental clinic accompanied by another person.

Seven days later

Removing stitches and cleaning the area of the surgery.

Four weeks later

Check-up to determine the progress of the healing process. Final denture placed.

Four to six months later

Check-up and cleaning implants, relining of the denture if needed.

Following the surgery there may be swelling and bruising. However, these usually go away after a few days. If you want to speed up the healing process you may apply cool packs to your cheeks. You could drink sage tea, which is known for its anti-bacterial effects. It is also advisable to refrain from using medication that may increase your blood pressure. Consumption of coffee or any caffeine drink is also best avoided. As soon as the local anaesthetic starts losing its effect you can start eating soft food. Clean your teeth using a rinsing fluid (Corsodyl) or by carefully brushing using a soft toothbrush. Smokers should refrain from smoking for 14 days after the intervention.

You should not feel any pain immediately after the surgery because you will have been given pain relief. You should not have any pain as we will provide you with a strict pain relieving regime. One week after the surgery any inconvenient effects should have disappeared, meaning that you can return to work.

In extremely rare cases sensitivity disorders in the chin area may result from an injury to the nerves. In addition peri-implantitis may develop in smokers and in people who have poor oral hygiene. The term peri-implantitis refers to periodontitis around the dental implant. This can be prevented by good oral hygiene. Always try to remove any food residues that may have deposited between your implants and palate or lower jaw gums.

Dental implants usually last many years, sometimes a lifetime. At London’s implant centre we give you a five-year warranty on your denture and a life-long warranty on the dental implants themselves (the warrantee does not include the procedure itself, but the cost of the implant material of every implant placed as the dental implant companies give us this warranty).

Generally speaking, good oral hygiene helps extend the durability of your restoration. Following your surgery you will be informed how best to clean your denture and how to use floss to remove food residue from underneath your fixed restoration. A professional biannual cleaning is mandatory for the warranty.

Due to the fact that bone augmentation usually is not necessary, the All-On-4™ concept may work out cheaper for you than conventional implants. How much this treatment will eventually cost you, however, depends on your individual situation. This can only be worked out on a case-by-case basis. Thanks to a treatment and cost plan you will know exactly what the cost of your treatment will be. Please refer to our fee page to find out more.

When making these calculations it has to be taken into account that professional dental cleaning, to be done twice a year, will also need to be factored in.

10 facts about All-On-4™: fixed teeth in one day

Those who suffer from tooth loss can have trouble chewing or feel uncomfortable with their appearance. With the help of the All-On-4™ method, you can enjoy life with a smile again. This treatment option allows the dentist to replace all missing teeth in a single session.

To do this, the dentist inserts the implants into the jawbone at a certain distance and at a precisely defined angle. Then he attaches the dentures — a bridge or a fixed prosthesis — which he screws onto the implants. Thanks to this stable base, patients can leave the dental practice with new fixed teeth on the same day.

Fact 1: All-On-4™ offers cost effective and uncomplicated dentures

Previously, a fixed bridge that would replace any lost teeth had to be anchored in the maxilla (upper jaw) to at least eight and in the mandible (lower jaw) to at least six dental implants. The All-On-4™ system uses only four implants per jaw. Since dental implants are expensive, the costs are significantly lower with this method.

Fact 2: implants without bone augmentation

If the jawbone is too thin the dentist must first set it up so that it can securely anchor the dental implants and dentures. Thanks to All-On-4™, elaborate bone augmentation procedures are now very often a thing of the past. Even with a very thin or narrow jawbone, we can still easily place the implants in most cases.

Fact 3: both jaws can be treated in one session

With All-On-4™, all teeth can be replaced in one session, even with complete tooth loss. This not only saves time but is also less stressful.

Fact 4: you do not have to wait for your new teeth

One of the biggest benefits of All-On-4™ is the fact that teeth can be replaced immediately after tooth loss. The prerequisite for this is that the gums are healthy and there are no inflammations. Therefore the method is especially suitable for patients who want to be able to socialise quickly and avoid prolonged treatment.

Fact 5: implants with All-On-4™ can last a lifetime

Implant-supported dentures usually last many years, sometimes for a lifetime. The more thoroughly you care for your dentures, the longer it stays functional.

Fact 6: All-On-4™ requires thorough oral hygiene

In order for you to enjoy your new teeth for as long as possible, a thorough oral hygiene is critical. Under fixed dentures, food particles that promote the growth of bacteria are often collected. This in turn can lead to inflammation, which can endanger the tightness of the implants. All-On-4™ patients should pay particular attention to their oral hygiene. It's best to have a professional implant cleaning done two to three times a year.

Fact 7: private insurance companies can participate

Privately insured people or patients with additional dental insurance will be reimbursed in parts or even for the entire cost depending on their contract. In case of doubt, you should enquire with your insurance provider before starting the procedure.

Fact 8: the All-On-4™ method is very safe

In general, this treatment is very safe and no side effects are expected. Only in extremely rare cases can the nerves on the chin be injured during the procedure, which can lead to sensory disturbances in this area. Patients who smoke or do not pay attention to the care of their dentures will increase the risk of peri-implantitis. This is periodontitis (advanced gum disease) on the dental implant.

Fact 9: the All-On-4™ method has existed since 1998

Since then more than 100,000 patients have received fixed new teeth in one day. The method is well tested and its effectiveness is scientifically proven.

Fact 10: certified Specialists for the best possible result
